Fee rows versus fee columns

For GiveCampus reporting.

The campaign all-gifts report emits a separate Donor-Covered Fees row. Another report's Donation Type column does not automatically reproduce those split rows. Use the fee amount column in that report, or transform an exported copy if separate labeled rows are required. Preserve gift identifiers and avoid counting a fee both inside a total and again as a separate row.
