Find the form appeal code in a custom gift report

For GiveCampus reporting.

The report field unique_identifier contains the source project's stored unique identifier/appeal code and is categorized as Gift Info. It differs from event_unique_identifier, which comes from the event, and from a form's display name. In Columns, search for unique_identifier or the school's/report template's alias for that field. The picker displays the applicable alias when configured, so the visible wording need not literally be Appeal Code. Confirm a sample against the form settings before mapping it to a CRM appeal field.
