Count distinct states and countries represented in a campaign

For GiveCampus reporting.

Use the campaign gift export with saved address state/country columns and the offline-gift report where applicable, restrict to the intended campaign/payment scope, normalize country/state labels and count distinct nonblank values. Review missing addresses separately rather than treating blank as another country. The map is a display of saved location data and applies its own rollups/deduplication; a ZIP alone is not proof the state/country field was saved.
