Anonymous and company-search controls on one form

In GiveCampus giving forms, available controls depend on your permissions and the interface used by your institution.

The standard Anonymous field is locked and cannot be removed as an ordinary custom question. For company matching, the form's Corporate Matching Gifts setting has Enable “Double the Donation” Company Search Field; clear that form-specific checkbox and save when the setting is offered. The school integration remains a separate configuration.

Custom fields can be created on the selected form with their own labels, field types and report-column identifiers. The creation workflow does not impose a special 20-field cap, but that is not proof an unseen membership questionnaire can be reproduced exactly. Review each proposed question's type, options, conditional behavior, required status and pre/post-payment placement against the actual editor, then test the whole form.
