Open Events → Events → select your event → Edit Event → Activities → select the activity → Details.

Add a dropdown custom question to the activity itself, with size options and the required flag if appropriate, when only its participants need to answer. Activity reporting can include those answers. This is data collection, not inventory management or automatic price calculation.
