How do I add first and last name to a giving form?

You can do that from form builder! Just head to the 'Forms' tab of your campaing or form and click 'Edit Form' at the top right. From here you can click the 'expand' icon:

Then select which fields you'd like to expand the name field to:

Finally choose 'I'm Done' at the bottom of the form.
